Poor Communication Leads to Medical Errors

Based on the estimate that 80% of medical errors are caused by communication breakdowns during patient care transitions, up to 78,000 patient’s deaths and over $13 billion in associated costs may be incurred.

Communication between care team members is critical.  In today’s era of mobile technologies and data sharing, there is no excuse for a lack of seamless communication between all care team members.  Furthermore, why should everyone need to be in the same room?  We can make a significant difference with improvements in care team efficiency and patient care delivery, by virtually integrating the care team, giving them the tools to get their work done, and allowing them to focus on delivering patient care rather than finding one another.
